RMKS/1.  EFFECTIVE IMMEDIATELY, NAVY WILL CONVERT TO AND USE THE NEW VERSIONS 
OF PRE-SEPARATION COUNSELING CHECKLIST FOR ACTIVE COMPONENT SERVICE MEMBERS 
(DD FORM 2648) AND PRE-SEPARATION COUNSELING CHECKLIST FOR RESERVE COMPONENT 
SERVICE MEMBERS RELEASED FROM ACTIVE DUTY (DD FORM 2648-1).

2.  TO ENSURE SAILORS FULLY UNDERSTAND THEIR ELIGIBILITY AND HOW TO ACCESS 
SERVICES AND BENEFITS, CAREER COUNSELORS PROVIDE A LEGALLY-MANDATED PRE-
SEPARATION COUNSELING SESSION TO TRANSITIONING SAILORS WHO HAVE BEEN ON ACTIVE 
DUTY FOR MORE THAN 180 DAYS.  THIS PRE-SEPARATION COUNSELING SESSION IS 
CONDUCTED NO LATER THAN 90 DAYS BEFORE THEIR PROJECTED SEPARATION/RETIREMENT 
DATE.  SAILORS BEING SEPARATED OR RETIRED DUE TO DISABILITY RECEIVE PRE-
SEPARATION COUNSELING REGARDLESS OF THE LENGTH OF ACTIVE SERVICE.

3.  TRANSITION/CAREER COUNSELORS WILL USE THE ONLINE TOOL, AVAILABLE AT 
WWW.DMDC.OSD.MIL/TAP.  TO GAIN ACCESS TO THIS TOOL, USERS SHOULD CONTACT THE 
OPNAV N135F POLICY ANALYST.  UNTIL USERS HAVE ACCESS TO THE ONLINE TOOL, 
SCANNABLE FORMS ARE AVAILABLE TO DOWNLOAD FROM 
HTTP://WWW.DTIC.MIL/WHS/DIRECTIVES/INFOMGT/FORMS/EFORMS/DD2648T.PDF.
ADDITIONALLY, TRANSITION/CAREER COUNSELORS AND OTHER AUTHORIZED USERS WILL 
HAVE ACCESS TO "THE TRANSITION ASSISTANCE PROGRAM (TAP) USER ACCESS GUIDE" AND
