RMKS/1. As of August 2024, OPNAV 1650/3 (Rev.SEP-2023) has been superseded
by OPNAV 1650/3 (Rev. AUG-2024).
All personal awards being originated after 30 September 2024 and being
submitted to the CNO Awards board must be submitted utilizing OPNAV 1650/3
(Rev. AUG-2024).
2. This mandate does not apply to those personal awards originated in The
Navy Department Awards Web Service (NDAWS).
3. The updated form can be accessed at:
https://www.forms.documentservices.dla.mil/order/. The site can be searched
by form number or title, using the drop down at the top. Once form has been
located, click the silver disc icon to download the form.
4. Utilize the amplifying guidance in the instruction section on the form to
verify form is filled out correctly. Discrepancies in the form will result in
delays in processing the award nomination and potential return to the
submitting command for rework.
