OF THE NAVPERS 1753/1 REV. 10/2011.  USE OF THIS UPDATED FORMAT IS CRUCIAL TO 
ACCURATE SEXUAL ASSAULT DATA COLLECTION EFFORTS.

10/2011 (SEXUAL ASSAULT INCIDENT DATA COLLECTION REPORT) TO THE DEPUTY CHIEF 
OF NAVAL OPERATIONS (MANPOWER, PERSONNEL, TRAINING AND EDUCATION) (N1) WITHIN 
10 DAYS OF INITIAL OPREP-3 NAVY BLUE OR OPREP-3 NAVY UNIT SITREP.  USE OF 
OLDER VERSIONS OF NAVPERS 1752/1 IS NOT AUTHORIZED.  THE CURRENT VERSION OF 
NAVPERS 1752/1 IS A FILLABLE FORM LOCATED ONLINE AT 
HTTPS://NAVALFORMS.DAPS.DLA.MIL/.  THE REPORTING COMMAND MUST COMPLETE AND 
AUTOMATICALLY E-MAIL OPNAV (N135) DIRECTLY AT opnav_sapr_report@navy.mil.  
THIS REPORT WILL HAVE LIMITED DISTRIBUTION TO ENSURE CONTROL OF SENSITIVE

3.  AS REQUIRED IN REF C, SUBMISSION OF AN OPREP-3 NAVY BLUE OR OPREP-3 NAVY 
UNIT SITREP FOR ALL INCIDENTS OF SEXUAL ASSAULT ENSURES THAT THE FIRST FLAG 
OFFICER IN THE CHAIN OF COMMAND IS NOT ONLY AWARE OF EVERY SEXUAL ASSAULT, BUT 
IS ALSO AWARE OF COMMAND ACTIONS TAKEN TO PREVENT SEXUAL ASSAULTS.  PER REF C, 
ALL SITREPS WILL INCLUDE THE COMMANDING OFFICER'S ASSESSMENT OF POTENTIAL 
EFFECTS THE INCIDENT WILL HAVE ON COMMAND CLIMATE AND ACTIONS THE COMMANDING 
OFFICER IS TAKING TO PREVENT FUTURE SEXUAL ASSAULTS.
