TRANSFER/REASSIGNMENT (PERMANENT AND/OR TEMPORARY) OF VICTIM(S) AND/OR
OFFENDER(S). (3) STATUS OF VICTIM'S COLLATERAL MISCONDUCT INVESTIGATION

REVISED AND AUTOMATED AND IS NOW A ONE-TIME REPORT REQUIRED WITHIN TEN DAYS OF
THE INITIAL OPREP/SITREP REPORT. NAVPERS 1752/1 IS A FILLABLE FORM LOCATED AT
HTTPS://NAVALFORMS.DAPS.DLA.MIL/ THAT THE REPORTING COMMAND WILL COMPLETE AND
AUTOMATICALLY E-MAIL DIRECTLY TO OPNAV (N135) AT
opnav_sapr_report@navy.mil. THIS REPORT WILL HAVE LIMITED DISTRIBUTION TO
